Human Relations Officer - Kuujjuaq

PROGRAM : Home Care - Kuujjuaq
STATUS : TEMPORARY full-time position (35 hours/week)
IMMEDIATE SUPERVISOR : Head of program Home-care
STARTING DATE : As soon as possible

FUNCTIONS :
Under the direction of the Head of Home Support Programs, intervenes (evaluation, follow-up, orientation and referral) with clients facing various social problems and loss of autonomy. Works with a multidisciplinary team, participates in case discussions, able to establish the needs of the clients with the multi-client evaluation tool and to make the intervention plan. Provides professional support to other team and community stakeholders. Works according to an individual, family or community approach and in collaboration with the multidisciplinary home support team.  Works to sensitize and mobilize the population around social problems.

Takes charge of curatorship and protection plan files. Provides OEMC training to the various stakeholders. Acts as a resource for an access window for SAPA files. Will have to travel to the village to provide support in more complex situations. Shares with the psychosocial team in place the after-hours guards.

Qualifications

Must have a university degree in the humanities or social sciences such as social work, criminology or psychology.
Member of ‘Ordre des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec’ or of his or her own professional association;
Have a minimum of 5 years of relevant experience, or have significant experience working with people with a loss of autonomy;
Fluency in spoken and written English and French; knowledge of Inuttitut is an asset;
Ability to work in a multidisciplinary team and interest in sharing knowledge and experience with colleagues;
Excellent knowledge of assessment tools for clients with loss of autonomy;
Committed to become a trainer of client assessment tools (training required);
Excellent knowledge of the laws of the Curatorship and the laws surrounding the application of programs to clients;
Sense of organization, initiative and autonomy;
Be available day, evening and night outside business hours;
Hold a valid driver’s license;
Available day, evening and night outside of business hours.

Conditions and benefits

SALARY : Min: 947.80$     Max: 1679.30$ for 35 hours
ISOLATION PREMIUM : 203.66$ /week
HIGH COST OF LIVING : 61.16$ /week
RETENTION PREMIUM : 114.99$ /week or more 

Any person recruited in Quebec more than 50 kilometers from the locality in which she is called upon to perform her duties, will be granted annual trips and lodging unit as per her family status, shared lodging if necessary.
