Systematic monitoring of clients (5 years old and over)
STATUS: Permanent full time position (36.25 hours/week) - Kuujjuaq
IMMEDIATE SUPERVISOR:  Head of Home Care program
STARTING DATE: As soon as possible


FUNCTIONS:
 Evaluation of patients diagnosed with chronical diseases (respiratory diseases, cardiac diseases, treatment of chronic wounds, etc.)

Coordinate the medical follow-ups for patients suffering from chronic within a multidisciplinary team

Establish the connection between the Liaison Nurse and specialists

Provide the required treatment to according to the patients’ needs

Promote and teach healthy lifestyles and self-care techniques

Refer clientele to the appropriate interveners

Work in collaboration with the medical team and the CLSC team

Organize weekly, monthly and annual wounds follow-ups

Participate in some public health activities (Vaccination, diabetes prevention)

Could share with the nursing team a 24/7 on-call service for home care clients and other services

 

This position is open to persons with a BAC (a DEC with relevant experience could compensate as long as candidates obtain a score of 90% on the evaluation grid that will be completed during the interview).

Qualifications

Member of the O.I.I.Q.

Bachelor’s degree in nursing. The absence of a diploma can be compensated by 3 years of relevant experience in systematic monitoring

Minimum of 1 year experience in systematic monitoring or with clients having chronic diseases

Knowledge of the PIQ

Enlarged role is an asset. Training can be provided later on

Experience in a intercultural environment

Knowledge of Inuit culture and Inuktitut would be an asset

Bilingualism in French/English is essential

Ability to work in isolated environment and within a multidisciplinary team

Be available day, evening and night, outside business hours

Valid driver’s license

Conditions and benefits

SALARY:  Min.:  $ 915.31 / week   Max.: $ 1 639.22 / week
ISOLATION PREMIUM: $ 191.93 or $ 338.29 / week, according to family status
HIGH COST LIVING PREMIUM: $ 59.14 / week or more, as per the family status
RETENTION PREMIUM:  $ 269.23 / week
CLSC PREMIUM:  $186.00 / week (If necessary)

Any employee recruited in Quebec more than 50 kilometers from the locality in which she/he is called upon to perform her/his duties, will be granted annual trips and lodging unit as per her/his family status. Transit or shared lodging if necessary
