STATUT: Temporary Full time (1 year) 36.25 hrs/week
IMMEDIATE SUPERIOR: Home care and Social services programs manager

STARTING DATE: As soon as possible

 FONCTIONS: Identify the health needs of people with chronic illnesses or mental health issues, as well as those with permanent or temporary loss of autonomy using the Multi-Client Assessment Tool (OEMC). Contribute to the methods of diagnosis, provide and control health services related to health promotion, disease prevention, treatment and rehabilitation and provide care according to a medical prescription. One-time support in various community and public health programs as needed.
 

Qualifications

Member in good standing of the O.I.I.Q .
Have a Bachelor of Science in Nursing or Bachelor's degree with 3 eligible certificates, including at least 2 nursing certificates
A minimum of one year of experience in community health in the various CLSC program
Knowledge of systematic monitoring programs in chronic diseases, mental health and home care
Enlarged role would be an asset
Work experience in remote areas would be an asset
Bilingual English / French, spoken and written, is essential and Inuktitut is an asset
Ability to work in a multidisciplinary team
Driver's license
Available to replace home care worker in different villages of the Ungava coast, as needed / maximum 1 month.

Conditions and benefits

SALARY: Min.: 915.31$ / week     Max: 1639.23$ /week
REMOTE PRENIUM: 191.93$ or 338.29$ / depending on family status
HIGHT COST OF LIVING: 59.14$/ week or according to family status
RÉTENTION PRENIUM: 268.23$/ week

POSTING PERIOD: From 2019-06-06 au 2019-06-20, inclusively

 Any employee recruited in Québec more than fifty (50) kilometers from the locality where she is called to perform her duties, will be entitled to annual outings and housing provided according to her status (transit or housing to share if necessary).
